Payments
- Payment in the form of cash, check or credit card (Mastercard, Visa or Discover) is requested for all services and/or co-payments at the time of the visit. Co-pays for office visits are usually higher for specialists, so each patient should check with their insurance carrier to determine this amount. Primary insurances will be billed on behalf of the patient for services rendered.
- Returned checks will be charged a $25 fee to cover processing and bank fees.
- Overdue accounts are subject to a $25 monthly fee.
- Following one missed appointment, it is office policy to charge a $30 fee for any appointments NOT CANCELLED at least 24 hours prior to the scheduled appointment time.
We recommend that all patients become well acquainted with the specifics of their insurance coverage.
- HMO (Managed Care) Patients: It is the responsibility of HMO patients to ensure that they have obtained a referral for all appointments with Family Dermatology & Skincare Center, if needed. The patient is financially responsible for any and all services rendered that are not part of the referral.
- PPO Patients: Most insurance companies consider dermatology procedures (such as skin biopsies or freezing off warts) to be surgical in nature. They will often apply these costs to your deductible.
- Insurance companies and Medicare will only cover procedures that are medically necessary. Skin tags, benign moles and sebhorrheic keratoses are not covered by insurance carriers. If you want these growths removed, we will remove these on a fee-for-service basis.
- All cosmetic procedures will be paid in full at time of service.
Refills
- We ask all patients to call their pharmacy to request any needed refills. Pharmacies will contact our office with the request, which will be addressed after review by Dr. Gyening.
